Any patient of Huntington Health Physicians can enroll by registering in-person at their physician’s office.

  • Patients age 13 and older will be required to register in-person at their physician’s office.
  • For patients age 12 and younger, a proxy account is established for parents or legal guardians.
  • Proxy access can be granted by patients ages 13 and older to parents or legal guardians.
  • Proxy access can be granted by the physician’s office if the patient has a diagnosis that significantly affects their decision making ability.
Parents or guardians of children or dependents that are patients will need to create an account for his/herself and from that account, request access to the child’s information. The request will be reviewed by the patient’s medical office and when approved you will receive an email invitation to become a proxy for your child. Accept the agreement to share your email. This is solely for the purpose of authenticating your online health account and your email will never be shared with any third party. Once you are signed in, enter the proxy password for your child/dependent. This is your child/dependent’s four digit year of birth (for example: 1980). The next three screens are documents which legally allow Huntington Health Physicians to provide you with an electronic copy of your child/dependent’s medical records.
You will be required to register for a Patient Portal account at your physician’s office. The physician’s office will verify your identity to ensure that your medical records are only released to you. You will then receive an invitation email to activate your Patient Portal account. You will be required to enter an invitation code that will be contained within the invitation email.
Yes, you have several options for logging into the Patient Portal. You can use your Facebook, Google, Yahoo, Windows LiveID, or Cerner login. Selecting to use any of these logins simply authenticates you to the Patient Portal. In no way is this information shared from the Patient Portal to them or from them to the Patient Portal. The Patient Portal does not share your login credentials and does not have access to your password. FollowMyHealth will NOT utilize the potential public information contained in these accounts such as pictures, contacts, friends, etc. and will NOT release and/or sell your information to third parties. You can also select to create a FollowMyHealth login by selecting the FMH icon on the login screen. Follow the steps to register for a FMH secure login.
After you have completed the registration through the invitation email your medical record information will begin to populate your personal health record automatically.
Maintaining your privacy is our top priority and we have taken the necessary steps to ensure we meet your expectations as well as all federal and state privacy laws. In compliance with federal patient privacy regulations, all data that is transmitted is encrypted during this process. Your personal data is also encrypted for your protection. Your login occurs over SSL (https), which in an industry standard for providing secure communication over the internet.
Similar to calling your physician’s office, messages are routed to the medical care team at your physician’s office. All messages that you send to your physician’s office becomes part of your medical record. You will receive a message or call within one (1) business day.
The medical staff from your physician’s office will respond to your appointment request within one (1) business day. Please call your physician’s office for same-day appointments or visit the Pasadena Community Urgent Care if you are unable to make a same-day appointment and need urgent care.

The Patient Portal should NEVER be used if you are having a life-threatening or urgent medical condition. Call 911 or go to your nearest emergency room.

Yes, in order to authenticate you, we must have a pop-up handles some communication between us and your chosen authentication method behind the scenes. If you are having trouble logging in, please turn off your pop-up blocker, and see if this solves the problem.
